This form is an aid to help you complete the IT risk questionnaire section of the contract request form in BuyWays and does not apply to UMass Chan. Please complete the IT risk questions correctly and to the best of your knowledge.


Upon submit, your completed form will be sent to you in email.  This document should not be sent to UPST in lieu of a contract request form. All contract requests must be submitted in BuyWays.
